Patients can access the your Express Care request forms via your Healthengine Practice Profile, or directly from your website. In order to set up your practice website button, you need to add the link to your website following the below steps.

When adding the Online Health Service Request button to your practice website, there are two options to add this link. You can either:
- Use a link to your own personalised text or image
- Use a Healthengine 'Online Medical Certificate' button (from the Medical Certificates preferences page)
- Use the Online Health Request Button (from the referrals or scripts preferences page)
Using the Healthengine Provided Button and URL Link for your Medical Certificate Request Form:
Navigate to Practice Admin > Medical Certificates > Preferences and scroll down to the Online Medical Certificates Button section.
You can use the same process to fine the Online Health Service Request.
There is a section here which displays the code snippet you are required to add to your website. Ensure your Practice ID number is correct. To find your ID, refer to the number at the end of the web browser URL when you are logged into Practice Admin, e.g. XXXXX in the image below. If you are unsure of how to use or where to place the code snippet, please copy it and forward it on to your IT support staff or the developers of your website and they should be able to assist.

Using Your Own Button and URL Link for Your Medical Certificate or Express Care Request Form (OPTIONAL):
If you wish to use your own styled button, you can also use do that. Copy the below snippet, but replace the SRS = section with your own source image. If you are unsure of how to use or where to place the code snippet, please copy it and forward it on to your IT support staff or the developers of your website and they should be able to assist.
